Club Scrap has some awesome book binding kits- complete with easy to follow instructions.

I've stitched several books and love the way they turn out! They might have a DVD on stitched books, but I'm not sure. I took a class at their retreat and learned how.
References :
look under the heading of book binding, you will need two sheets of very heavy cardboard like matte board or even two thin sheets of plywood, then fabric or something to cover it, even leather or leather look vinyl would work just fine, then cover the inside of the cover maybe with heavy card stock, you need an ice pick, a heavy needle and heavy thread like a waxed cord they use to sew leather, and pliars to pull the needle through every stitch, because it gets really hard to do, then your pages need to be between and pierced with the ice pick too so the holes match the covers, double stitch it either by using two needles on the end of the same piece of thread and stitch one through back to front and one through front to back right after, and continue this pattern all the way up , or stitch it from the bottom to top then back down to the bottom so there are no open spaces in the binding without thread, that means each hole is gone through twice to give it extra strength, you will need to make sure you pull that string tight, very tight, you will also want to score the cardboard at the area where the holes go through before you cover the pieces so that the book cover will bend open neatly. book binding is becomming a lost art, good luck.
